Weight Management

We are working to identify patients who live with obesity and refer them onto NHS funded weight loss support programmes.  These include Slimming World and Ealing Weight Management Service and the NHD Digital Weight Management programmes. Links through to these organisations are below.

Our clinical pharmacists are working with patients to identify those who are ready to make behavioural changes and referring them onto weight management programmes. They will be sending out text messages to those patient they have identified as falling into the obese weight category and asking if they would be happy to to discuss and progress with a referral onto the programme.

We will also send out a link to all patients to give us their most up to date weight and height so that we can have a recent recording of their BMI.
